The Children's Commissioner has got some strong views on what can be done to fix the system.
Russell Wills says if he could change one thing, it would see that CYF stays focused on the immediate safety of children, and that it makes sure children are at the heart of everything they do.
Secondly he says CYF needs to have enough social workers and that they train them properly to give quality care.
