A man threatening a doctor at a Sydney hospital is said to have shot a police officer and a security guard.

9NEWS are reporting the police officer lost control of his gun during a struggle with the man threatening the doctors around 10.30pm last night (local time) at Nepean Hospital's emergency department in west Sydney.

"During a struggle, a police officer lost control of his firearm. Subsequently, the police officer and a security guard were shot," New South Wales police said in a statement.

A police officer and a security guard were shot in the struggle.The police officer was shot in his upper leg and the security guard shot in the calf, according to 9NEWS.

The men are currently undergoing treatment in Nepean Hospital with the police officer in a "critical but stable condition" according to NSW police and the security guard is stable.

The incident is believed to have happened inside the hospital's emergency department. The 39-year-old suspect was arrested and is being questioned by police.
