A US mother's video, farewelling her son before handing him over to his adoptive family, has gone viral.

The video posted to YouTube early in 2017 shows 18-year-old Hannah Mongie explaining to her sleeping baby why she was following through with the adoption.

The reason behind her decision is heart-breaking - early in her pregnancy, Ms Mongie's partner and the baby's father, Kaden Whitney, died.

"Two days after he heard your heart-beat, his heart stopped," she said.

"From the day your daddy died, I had a hard time even considering placing you with a different family, when you were my last piece of Kaden.

"But one day, I just woke up and knew you were supposed to be with someone else."

Ms Mongie speaks highly of his new family and adoptive mother Emily Marsh, saying they're lucky to get to raise him.

"I fell in love with them and they were beyond anything I could have asked for," Ms Mongie said.

"I'm really excited for her, because she gets to love you so much and she gets to raise you. She's already the best person I know."

Ms Mongie pursued an open adoption, and regularly posts photos of her and the baby, now named Taggart, together on her Instagram account.

The video has received more than 760,000 views since it was posted, with thousands of comments in support of Ms Mongie.

"Your decision is one of the strongest ones I've ever heard," one commenter said.

"Your decision is a decision out of love."
