Home and Away star Jessica Falkholt has died after being injured in an horrific car accident that killed three members of her family last month.

She was 29.

Falkholt's parents were both killed in the crash, which also left her sister with fatal injuries.

She remained fighting for her life in Sydney's St George Hospital, where she underwent multiple surgeries.

Her life support was turned off on January 12 and the hospital has now confirmed her death.

"Jessica passed away from serious injuries sustained in a motor vehicle accident on December 26," St George Hospital says in a statement.

Police believe the crash occurred when a large vehicle crossed the centre line and crashed head on with the Falkholt family's car.

Ms Falkholt and her sister Annabelle were pulled from the crash, as the vehicles went up in flames, but her parents died in the burning car, along with the 4WD's driver. Annabelle, 21, died three days after the accident.

Police are continuing to investigate the cause of the crash. Australian media are describing the driver of the other vehicle as a "serial traffic offender" who was returning from a methadone clinic when he collided with the Falkholts' car.

Ms Falkholt played Hope Morrison in Home and Away, and appeared in a number of episodes in 2016 before leaving the show. She recently starred in Harmony.
