Heart-breaking details of rapper Mac Miller's death revealed

The report cites McCormick's cause of death as "mixed toxicity" from a combination of cocaine, alcohol, and fentanyl - the deadly opiate responsible for a string of accidental deaths across the US.

TMZ reports the document describes the moment the 26-year-old's assistant discovered him unresponsive and in a "praying position" on his bed, kneeling forward with his head on his knees.

The assistant called emergency services from McCormick's LA home, but later said he was already "blue" during the call, according to the gossip website.

A police investigation discovered a bottle of prescription drugs, an empty liquor bottle and "linear white powdery residue" at the property, as well as a rolled up $20 note in McCormick's pocket.

The coroner also detailed all of the 'Self Care' hitmaker's tattoos during the autopsy, including one that reads "only so much time left in this crazy world", and another that says "we are confused".

McCormick died on September 7, after a lengthy battle with substance abuse which he often referred to in his songs.

At the time of his death, a source told People magazine he had spent the night before watching football with friends.

Ex-girlfriend and popstar Ariana Grande has paid tribute to McCormick several times since his death; with an emotional Instagram post and a mention on her latest single 'Thank U, Next', calling him "an angel".
