The one-Test All Black struggled with concussion since a head knock against the Highlanders last May.
But on Saturday he started at fullback for University against Otorohanga in Waikato club rugby.
The 26-year-old played the first half before being subbed off in a 30-20 win.
The Chiefs face the Sunwolves and Reds in the next fortnight and after a successful hit out at club level, it could be the perfect opportunity for Ngatai to make his long awaited return to Super Rugby.
