Dame Valerie, 33, posted a photo of the two of them together to Instagram on Sunday.
"My heart hurts," she wrote. "Rest in Love my beautiful Grandma. Say hi to Mum for me."
Adams' mother Lilika Ngauamo died of cancer in 2000, when the shot putter was just 15.
In 2015, Adams told NZME that she was inspired by her grandmother's fight for her life, and that it helped her during her own recovery from surgery.
"Grandma is the toughest person I know," she said.
Mrs Otuafi underwent chemotherapy in the United States in 2015.
