A three-month-old baby boy has become the fifth fatality following Friday's deadly rampage in the Melbourne CBD.

The infant was among five people left in a critical condition after a driver ploughed a car into crowds on busy Bourke Street.

A 10-year-old girl was also killed, along with two men and a woman who also died at the scene.

Police Commissioner Graham Ashton has warned that the death toll may still rise, with four people still in hospital with critical injuries.

The driver is being treated in hospital for non-life-threatening injuries, after police rammed his car and shot him during a chase.

Described as a man in his 30s, he did a number of burnouts at the main intersection before driving his car into the shocked crowd.
